Speech therapy SOAP note example with targets and cueing.

Keep language, speech-sound and communication session details visible in a clear, editable SOAP note.

Start with the facts from the visit

Speech-language pathology documentation often needs to state the target, number of opportunities, observed performance, cue type and clinical interpretation. Whether treating adults or children, a note should distinguish prompted performance from independent success. The following fictional case illustrates organization only; use your actual data and your practice's requirements.

Name the treatment target

Identify the sound pattern, language task, communication skill or other goal addressed during the session.

Separate accuracy and support

Record trials, accuracy, cue type, modeling, AAC supports and level of independence as actually observed.

Explain the response

Interpret the performance in context of the task and support, without claiming mastery or generalized improvement from one example.

Plan the next practice level

Describe how the clinician intends to adjust the target, supports or task demands based on observed performance.

Worked SOAP note example

The following invented case illustrates how to separate reported information, observed interventions, interpretation and a next step. No clinical facts should be copied into a real chart.

Subjective

Caregiver reported the child attempted more two-word phrases during play at home.

Objective

During structured play, child produced two-word requests in six of ten opportunities following a model and visual cue; initiated two additional requests without a model.

Assessment

Observed requests remained more frequent with modeling than without it. Continued work will focus on increasing independent functional requests while monitoring generalization.

Plan

Continue play-based requesting tasks and systematically reduce modeling as supported by the child's response.

Questions clinicians ask

Can it handle trial-by-trial counts?

Include your actual trials and accuracy in the input. Confirm the counts and conditions are copied correctly.

Can SLPAs use it?

Where permitted, assistants can draft session descriptions, subject to the SLP's supervision and local documentation rules.

What about adult cognitive or dysphagia therapy?

The same factual drafting process can apply; enter the correct observed targets and follow all relevant assessment and safety requirements.
