Therapy assistant documentation: clear notes within your scope.

PTAs, OTAs, COTAs and SLPAs need accurate visit records without implying independent assessments or authority their role does not include.

Understanding the documentation task

Therapy assistants provide treatment and gather session observations as allowed in their setting. Documentation responsibilities, supervision and cosignature rules are jurisdiction- and setting-dependent. A drafting tool can organize what happened, but it does not expand a clinician's scope or determine who must sign.

What an assistant can accurately record

State the activities performed, cueing, assistance, recorded measures and response observed during the treatment. Attribute patient or caregiver reports accurately.

What requires scope awareness

Avoid independent evaluation, diagnosis, goal changes or other clinical decisions outside the assistant's authorized role. The supervising professional and site policy govern assessment language and sign-off.

What the supervising therapist may need

Document the intervention, observed function, response and concerns clearly enough for the supervising clinician to review or cosign as required.

Documentation handoff

Use clear dates, objective facts and any required communication to the supervising professional. An AI-generated draft is not proof that supervision occurred.

Common questions

Can a COTA use NoteyDoc?

Yes for drafting factual session summaries where the organization permits it. Local OT supervision and review rules still apply.

Can PTAs or SLPAs document independently?

That depends on applicable rules and site policies. NoteyDoc does not authorize an independent evaluation or override a required cosignature.

Can the tool apply electronic signatures?

No. Complete signatures and required approvals within your authorized documentation system.
