Documentation guides / Therapy SOAP note workflows

How do travel therapists handle SOAP notes across multiple agency EHRs?

If you rotate between clinics, home-health agencies or short contracts, you may use a different record system every week. A separate summary-to-SOAP drafting tool can keep the writing step familiar, even when your EHR changes.

NoteyDoc Unlimited: $15/month for unlimited SOAP note drafts. Check full plan details.

One writing workflow, separate agency records

You finish the visit, record the actual treatment details in a short summary, generate an editable SOAP draft, and review it. Then you place the reviewed content in the agency's approved EHR and follow that site's required fields and signatures. This keeps the drafting step consistent while your agency's EHR remains the place for required fields, review and signatures.

Check approval before using the same outside tool at different organizations. Each agency may have different rules for approved vendors, browser extensions, devices and patient data. Confirm those rules first, especially when moving between organizations.

How a travel PT, OT or SLP might use it

Travel PT in outpatient rehab

Summarize gait activities, exercise dosage, device use, assistance and the patient's response. Edit the SOAP draft, then enter it into the facility's chart.

Travel OT in home health

Capture daily activity performance, transfers, adaptive equipment and caregiver training after the home visit. Draft a SOAP note, then complete any separate agency-required forms.

Travel SLP in schools or clinics

Record goals, trial counts, prompts and observed responses. Review the SOAP sections and document in the school's or clinic's required system. School reporting and service logs remain separate.

Keeping the same note workflow at each assignment

Ask: Does the website work on your permitted device? Does the Chrome extension work in this particular EHR, on this particular browser? Can you paste the reviewed draft without losing important formatting? Are the external AI workflow and privacy agreements approved at each agency? Try your usual note workflow in each permitted EHR so you know what to expect.

NoteyDoc Unlimited is $15/month for unlimited SOAP draft generation. It can be especially useful when your main bottleneck is turning shorthand treatment facts into clear SOAP documentation. Use the resulting note through each agency's approved charting process.

Example after an assignment visit

Fictional rough summary: Outpatient PT, gait training 100 feet using front-wheeled walker, contact-guard assist for turns, two verbal cues for foot clearance, reported fatigue after stairs, plan to continue stair practice.

Put patient-reported fatigue in Subjective. Include the 100-foot distance, device, assistance and cues in Objective. Write an Assessment that stays within the observed performance, and a Plan based on what was actually decided. Do not add diagnoses, measurements or improvement claims that were not provided.

Questions from multi-agency clinicians

Does the $15 plan follow me between agencies?

The NoteyDoc account offers unlimited SOAP drafts on the paid Unlimited plan. That does not override each agency's restrictions on external software, PHI processing or extension installation.

How do I handle notes at five different agencies?

Use the same brief-summary workflow where the tool is approved. Review each generated SOAP draft, then add it to the appropriate agency EHR and complete that agency's required review and signature steps.

Can I use it on a work phone?

The website can work in a supported mobile browser, subject to device, network and employer restrictions. Browser extensions are not supported by standard mobile Chrome.

Can a traveling COTA, PTA or SLPA use it?

They may use a drafting aid where permitted, but scope, supervision, review and cosignature requirements still apply in the state and setting where care is provided.
